Participation agreement

Review draft v1.3 · 18 October 2026 · Adults aged 18 and over

1. Who this agreement is with

This draft is for DTL Coaching and Dale Thomas Lambrick (together, “DTL”) and the adult participant named below. The contracting business’s full legal details and final agreement wording remain subject to review before coaching starts. It covers following the coaching and training programmes agreed with DTL. DTL provides programmes only; organised rides, group sessions and in-person supervised activities are not included. Coaching scope, fees, cancellation rights and any event terms must be agreed separately; this form does not replace them.

2. Understand the activity and its risks

Endurance training, cycling, running, swimming, indoor training and participation in events involve inherent risks. These include falls, collisions, traffic, uneven or slippery surfaces, equipment failure, weather, exposure, dehydration, fatigue, overexertion, illness and injury. Open-water swimming also involves cold-water shock, currents, poor visibility and drowning. Serious injury, permanent disability or death is possible. Risks cannot all be eliminated, including when reasonable care is taken.

I choose to participate voluntarily, understand the reasonably foreseeable inherent risks described above and accept those inherent risks. This acceptance does not extend to liability that cannot lawfully be excluded, or mean that I accept negligent coaching or unsafe organisation.

3. Coaching and training programmes

DTL will exercise reasonable care and skill in providing agreed coaching. A training plan is not medical advice, diagnosis, treatment or a guarantee of fitness, race completion, safety or any performance result. Programme-based coaching does not mean that a coach is accompanying, supervising or monitoring me during any session. I arrange my own training locations and any activity-specific supervision or safety support needed.

I will provide accurate information about my experience, current training, relevant health restrictions and readiness. I will tell my coach promptly about changes that may affect safe participation, including illness, injury, medication effects, pregnancy or medical advice. I will seek advice from an appropriately qualified health professional where needed and will not train against medical advice. DTL may pause or adapt training while suitability is reviewed.

I will use judgement about my condition, recovery, the location, weather and equipment before each session. I may reduce, stop or decline any session that feels unsafe. I will not push through concerning symptoms or follow a target at the expense of safety. I will stop exercise and seek appropriate medical help for concerning symptoms such as chest pain, fainting or unusual severe breathlessness; in an emergency I will call 999 or 112. I will discuss missed or unsuitable sessions with my coach rather than increase the workload without agreement.

4. Equipment, environment and other providers

I will use equipment suitable for the activity and my ability, check its condition, follow facility rules and comply with applicable road and access rules. For outdoor cycling I will wear a correctly fitted cycle helmet, use required lights and carry appropriate clothing, hydration, repair equipment and a means of contacting help. I will avoid riding while impaired by alcohol, drugs or fatigue. I will follow the Highway Code and make my own safe decisions at junctions and around other road users.

I will not swim alone in open water and will use an appropriate supervised venue or agreed safety arrangements. Separate venues, races and other providers may have their own safety requirements and terms. I will check my own insurance needs, including personal accident, equipment, travel and third-party cycling cover. No personal insurance cover is promised by this form.

5. Legal rights and responsibility

Nothing in this agreement excludes or restricts liability for death or personal injury caused by negligence, fraud or fraudulent misrepresentation, or any liability that cannot lawfully be excluded or restricted. My statutory consumer rights are preserved, including the right to services performed with reasonable care and skill.

I remain responsible for my own negligent or unlawful acts and for taking reasonable care of myself and others. DTL is not responsible for a loss to the extent it was not caused by DTL’s breach of a legal duty or contract. There is no blanket release of negligence, no obligation to reimburse DTL for its own negligence and no financial liability cap in this draft. Responsibility for an incident must be assessed on its facts and under the applicable law.

6. Readiness, review and changes

This short readiness check is not a medical assessment or a validated medical clearance tool. An answer indicating a concern means participation must be discussed with DTL before starting; medical advice may also be needed. Signing is not clearance to start a training programme. This adult form must not be used to enrol anyone under 18.

I will update DTL when circumstances change. DTL should review this declaration at least annually and before a materially different activity. Material changes to the agreement require a fresh acceptance. This agreement does not cover undisclosed future activities automatically.

7. Personal information and records

Submitting this form stores my contact details, emergency contact details, readiness answers, acknowledgements and typed signature privately on the DTL website. The record includes the exact agreement wording, version and the server receipt time. Access is restricted to Dale and his authorised website administrator. My email address and identity are not independently verified by this form.

I have read the athlete privacy information. Health-related readiness answers are processed with my explicit consent, separately from acceptance of these terms. I can contact Dlambrick5@gmail.com about access, correction, deletion or withdrawal of health-data consent. Withdrawal does not undo previous lawful processing; Dale will explain whether coaching can safely continue without that information. No marketing or photograph consent is included.

8. Questions and applicable law

I may ask DTL questions before accepting and should keep a complete copy of the exact agreement I accept. This draft is intended for activities in England and Wales and is subject to their law, without removing mandatory consumer protections or court rights that apply to me elsewhere. Participation abroad or in other jurisdictions requires a separate suitability check.
