Athlete information

DTL Coaching · Contact: Dale Thomas Lambrick · Dlambrick5@gmail.com

This notice describes the website declaration inbox. The coaching agreement remains a review draft and final business and retention details must be confirmed before coaching begins.

What is collected and why

The form records your name, email, phone, emergency contact details, readiness answers, typed signature, accepted wording, agreement version and server receipt time. Dale uses these to review your request for coaching, discuss readiness concerns and keep evidence of your declaration. Provide only the requested information, without detailed diagnoses or medical records.

Health-related answers

Your explicit, separate consent permits DTL to store and review readiness answers that reveal health information. You can withdraw it by contacting Dale. He will explain whether coaching can safely proceed without that information. Withdrawal does not undo previous lawful processing. Other contact and administration information is used to take steps you request towards coaching and to maintain the declaration record.

Where it is saved and who can see it

The declaration is stored in the website’s hosted database, supplied through the Sites hosting service and Cloudflare infrastructure. It is not saved to a public webpage or Google Drive. Access to the private inbox is restricted to Dale and the authorised website administrator, Julian Poole, through signed-in accounts. Operational hosting providers process data to run the service. The form does not verify legal identity or email ownership.

Notifications and your copy

Dale’s inbox displays new submissions. Email notifications and automatic email receipts are not connected at present. After saving, you may choose Notify Dale on WhatsApp. This opens your WhatsApp with your name, submission reference and the private inbox link; you must tap Send to deliver it. Readiness answers and emergency contact details are not included in that message. You can download or print your complete declaration after successful submission. When the WhatsApp sender is connected, the website may send Dale a short form-completed alert containing your name, submission reference and private inbox link. Readiness answers and emergency contact details are not included. No readiness answers are put in a public notification.

Optional plan adjustments and WhatsApp preferences

The optional adjustment form stores only the dates you ask to change, your comments and contact details in Dale’s private workspace. Athletes whose published plan suits them do not need to submit a form. WhatsApp service-message permission is recorded separately at onboarding; a change request does not set, withdraw or refresh that permission. Please avoid detailed medical information. Updates are requests for Dale to review; they do not change a TrainingPeaks programme automatically. Your identity and mobile ownership are not independently verified by this form, and Dale must check your contact and permission before sending plan updates.

If you opt in, DTL may send service messages about your training plan and availability through a WhatsApp Business messaging provider. The connection is not active yet. You may decline without losing access to the form, and may withdraw by contacting Dale at Dlambrick5@gmail.com. No marketing permission is requested. Automated message drafts may be kept in the private message outbox while the sender connection is being configured. Information sent through WhatsApp is processed by the messaging provider and WhatsApp/Meta.

Retention and your rights

Records are kept for coaching administration and documented legal or insurance needs. Ask Dale for the applicable retention period; the final retention schedule remains to be confirmed before coaching starts. You may ask for access, correction, deletion, restriction or a portable copy, and raise an objection where applicable. Some records may need to be retained for legal reasons. Contact Dale to make a request or withdraw consent.

You can complain to the UK Information Commissioner at ico.org.uk. No marketing or photograph permission is requested. Only essential sign-in functionality is used for the administrator inbox; form answers are not kept in browser storage.
